Welcome to the 2023 program year! We are greatly looking forward to seeing our students during our four Saturday Academy sessions (April 29, May 6, May 13, May 20, & June 3) and then a few weeks later for the start of our summer program on Monday, June 26th.
Registration for NEW families must be completed by May 1, 2023
Registration for returning families must be completed by May 20, 2023
STEP 1: TEACHER EVALUATION FORM FOR RETURNING STUDENTS
(For returning students only; newly admitted students completed this step during the admission process.)
Copy the Teacher Evaluation Form link and email it to your child’s teacher AS SOON AS POSSIBLE. Teachers get extremely busy as the end of the school year approaches, so please do this right away! STEP 2: REQUIRED REGISTRATION FORMS FOR ALL STUDENTS (new & returning)
Click the blue titles to go to each of the three forms, complete and submit each one. You will receive a notification if it was successfully submitted. If not, please return to the form to find the error, then return to this page to complete the rest of the steps.
Horizons GFA Student & Family Contract
One form to be completed per family. Students and parents/guardians are to complete this form together.

STEP 3: PAY ANNUAL REGISTRATION FEE
A $30 per student registration fee is due by June 1st.
We accept checks, money orders, and online payments. Do not send cash!
